Dear Members and Followers,

The deadline to complete the GOCCNJ Palliative Care survey is November 30, 2023. Only one member per organization/service line need complete the survey. Please see the request and survey link below.

Dear NJPC APN Consortium Members:

The Goals of Care Coalition of NJ (GOCCNJ), with funding from the New Jersey Commission on Cancer Research (NJCCR), recently launched a survey to gain a better understanding of our state’s capacity to deliver palliative care to people in New Jersey with serious illness. GOCCNJ has made changes to the questions in order to simplify the survey and make it easier for palliative care program administrators to complete. The revised survey is available using the link below. In addition, a PDF copy of the survey questions is attached.

As you may recall, the data from this survey will be shared in a published report about the challenges and opportunities to improve access to palliative care services across the state. This will also be the source from which GOCCNJ will develop a NJ Directory of Specialty Palliative Care Providers that will allow healthcare professionals to refer patients to palliative care services and patients, families, and caregivers to identify specialty palliative care providers in their community.

We encourage you to please complete this survey. With your help, we will be better positioned to identify gaps and address the challenges and barriers that prevent New Jerseyans with serious illness from receiving the best possible care. Please complete by November 30, 2023. Thank you in advance for your participation!
