Palliative Care Provider Survey – Please Complete by Oct 31st

Dear NJPAC APNC members:

 Our friends at the Goals of Care Coalition of NJ (GOCCNJ), with funding from the New Jersey Commission on Cancer Research (NJCCR), are asking for your participation in a survey to help gain a better understanding of our state’s capacity to deliver palliative care to people here with serious illness. The data from this survey will be shared in a published report about the challenges and opportunities to improve access to palliative care services across the state and will be the source from which GOCCNJ will develop a NJ Directory of Specialty Palliative Care Providers that will allow healthcare professionals to refer patients to palliative care services and patients, families, and caregivers to identify specialty palliative care providers in their community. We urge you to please take the time to complete this survey. With your help, we will be better positioned to identify gaps and address the challenges and barriers that prevent New Jerseyans with serious illness from receiving the best possible care.
