We are proud to be a part of GOCCNJ and support their mission to imrove EOL care in New Jersey. Please take a moment to review these recommendations for improved ACP and POLST in long term care settings.
Kudos to Katie Demarco and Colleen Bell our VPs for Community, Legislation and Health Policy for representing our Consortium so ably on the GOCCNJ and making sure our name and logo were included.
