Information on placing a hold on DNR order

I'm looking for current practice/policy for holding/rescinding a DNR order when a patient goes to the OR (if they elect to rescind the DNR). Specifically, when does the DNR order go back into effect? At my former institution the DNR automatically went back into effect once anesthesia released the patient. At my current institution there is "hear say" (since I can't find a policy) that the DNR remains rescinded for 24 hours after the surgical procedure.

Our policy states it is rescinded at time or admission to the OR of prior to procedure and then automagically reinstated, either after discharge from PACU , after discharge from the procedural department or after the patient has fully recovered from the effects of anesthesia.
I am currently on a sub committee at my facility looking into standardizing color coding banding (purple) for patients with DNR designation. Not much out there in the literature, plus Joint Commission does not have a position on the topic. Are there any policies/practice currently in place in any of your facilities?
