You will work independently, seeing patients for 7 contact hours per day (including travel), with your travel time fully accounted for within your schedule. After completing our ramping process, you will be responsible for improving patient health outcomes, reducing hospital visits, and closing health gaps for high-risk patients.

You will receive a daily schedule with visit time frames and locations, allowing you to manage your day efficiently. This role requires comfort with visiting various home environments and autonomy in patient care.

Provide primary care to patients in their homes, focusing on minimizing hospital visits and closing health gaps.

Work with your POD team to develop care solutions for high-risk patients.

Document visits using Aprima (transitioning to Athena in May 2025).

Attend team meetings on Tuesdays to discuss patient care and improvements.

Ensure patient safety, privacy, and compliance in all home visits.

Nurse Practitioner (NP) License in New Jersey.
